Two Montgomery County dental practices said Friday that a third party may have gained unauthorized access to a computer server containing patient information.
King of Prussia Dental Associates and its affiliate, Pediatric Dentistry of Collegeville, said in a statement that they detected irregular activity on the server around early June and took immediate action, hiring a forensics computer firm and contacting law enforcement.
On June 23, the forensics firm determined that unauthorized access may have been gained to the practices' computer network.
The company said the affected server was taken offline immediately, and the investigation has revealed no evidence that "any information was acquired or used inappropriately."
"However, the server contained patient information, including name, social security number, date of birth, home address, phone number, account number and treatment records," the statement read. "KOP Dental recommends that patients remain vigilant, and is offering credit monitoring and identity theft protection services."
On Friday, KOP Dental began mailing notification letters to patients whose personal information who may have been accessed.
Patients with questions can call and enroll in free ID Experts® MyIDCare identify and credit monitoring services at 844-575-745, from 9 a.m. to 9 p.m. EDT from Monday to Friday using the Access Code provided in their notification letter. The deadline to enroll is Dec. 9, 2016.
KOP Dental said it is working to enhance its network security to prevent unauthorized access in the future.
